Family Navigator (Part-Time)
Job Description: The Family Navigator assists individuals and families in finding the appropriate mental health providers, treatment facilities, and supportive services needed for mental health and addiction recovery. Our navigators provide compassionate listening, information, and education, not direct care.
Job Responsibilities:
- Put together individualized plans of available mental health resources and pertinent educational information for our clients
- Follow up with clients to confirm they have connected to a resource
- Provide documentation on client connections as outlined in the policies and procedures
- Develop a thorough understanding of the available resources for our community
- Build strong working relationships with the community resource providers
- Find creative solutions to client barriers to care
- Provide education on treatment, care options, and expectations for the specific situation
- Listen to our Hope Pieced Together Podcast so appropriate episodes can be shared with clients
- Build awareness in the community about the resource that Mosaics of Mercy provides
- Identify gaps in mental health and addiction resources
- Attend events as needed to represent Mosaics

- Familiar with use of Google Docs, Drive, etc.; or willing to be trained
- Licensed or certification in an area related to mental health or addiction
- Spanish speaking a plus
- Proficient in computer use
