Workshop Waiver

  • At a mosaic workshop, I understand that by participating I will be working with pottery and glass shards and I agree to exercise caution while handling these shards. If I choose to create pottery shards, Mosaics of Mercy will provide me with safety glasses. I understand I will also be working with messy grout and mosaic supplies and will wear appropriate clothing or covering.

I agree to wear any and all personal safety equipment that is provided to me during the session. As a participant in a mosaic session, I release and agree to hold Mosaics of Mercy harmless from any and all liability and hereby assume all risk and responsibility and waive any claim or cause of action of any nature arising while participating in a creative mosaic session. I further agree to indemnify and hold harmless Mosaics of Mercy, its agents, employees or contractors, whether paid or volunteer, against any and all claims which may arise from any injury while participating in this program.

I understand photos may be taken where I am visible in a group setting and allow Mosaics of Mercy to use these for social media and website purposes.

The mosaic workshop is not a counseling session and is not meant to replace counseling or be a substitute for it. It can be a time of personal artistic expression and can be an outlet or space for healing to some. Mosaics of Mercy can provide additional mental health resources as desired following the workshop. Session attendees are not required to share personal information with the group. if an individual decides to share personal information, we ask that all members of the group, including Mosaics of Mercy staff and volunteers, keep that information confidential and only discuss it in the group setting.

I have read and understand the above policies and agree to comply with them.(Please write clearly and legibly! Thank you!)
